Flat Rate Fee
After the consultation call, you will receive a free quote for a flat rate psychological assessment fee.
A nonrefundable $500 deposit is required to book the intake appointment and will be applied toward that fee.
Insurance
This practice does not currently accept any insurances.
A superbill will be provided for families seeking possible out-of-network reimbursement. Families are responsible for determining their insurance eligibility, benefits, reimbursement rates, and submitting claims directly to their insurance company.
Uninsured and self-pay clients have the right to receive a Good Faith Estimate of the expected cost of services, as required by law.
Scheduling & Cancellation
There is currently no waitlist, and most families can be scheduled within 1-2 weeks for an intake appointment.
Appointments canceled or rescheduled with at least 24 hours' notice may be rescheduled without issue. Cancellations made less than 24 hours before the scheduled appointment may not be eligible for rescheduling and are subject to the discretion of the practice.
If your child or anyone in your household is ill, please notify the practice as soon as possible. Illness is considered an excusable reason for cancellation or rescheduling, and we kindly ask that no one attend appointments while sick to help protect the health and safety of all families.
